How To Find Out if You Qualify
Evaluation Procedure
 

If you decide to undergo our evaluation process, please contact us directly by email, phone or fax before proceeding.  We will provide additional instructions.

 

*** Please be advised that as of November 1, 2000 we are no longer accepting evaluation information by mail, contrary to any additional information you may have read or received. ***

We have experienced unfortunate delays with the evaluation process by mail in the past.  Therefore, all evaluations will be done by making an appointment with our medical staff for an on-site visit.  Although this will require you to make travel arrangements, and to have all of your relevant medical information available for our staff when you arrive, we believe that overall you will find this process more responsive to your individual needs.

On very rare occasions this rule may be waved by the Medical Director following a direct discussion of your needs. However, it should not be assumed that this permission will be granted.  In any event, you will eventually need to visit the Clinic personally for the complete physical evaluation, since no final evaluation will be provided in absence of the patient.  Under no circumstances should you send any original documents, since we cannot guarantee their safety.



 
Please be ready to provide the following information when you arrive:
  1. A completed "Initial Contact Information" form. (Provided as a separate attachment once we have made contact).
  2. A complete set of medical reports related to the injury.
  3. A current MRI or CT scan (within the past six months) that clearly images the current status of the soft tissue in the spinal cord. (Without this information we cannot complete the evaluation.) If metallic implants are associated with the injury, a CT Scan with Dye Contrast may be required rather than an MRI, since metal objects may cast shadows that render MRI images unusable for diagnostic purposes.
  4. A set of X-rays that show the current status of the bony spinal canal.
  5. A second set of X-rays the show the condition of the injury prior to any treatments you have received.
  6. A current Physical Rehabilitation Report, which indicates any progress or regression that has occurred since the injury.


Please be advised that the evaluation fee is payable in advance of your visit to reserve that date for you. This fee includes the "evoked sensory potential" examination with a medical neurologist who specializes in the application of this procedure to spinal cord injuries. In addition your assessment will include orthopedic, neurosurgery and rehabilitation evaluations, as well as other tests deemed essential by the physicians.

The evaluation fee will be discussed following your initial contact with us. On occasion an additional test may be required, such as a new MRI, which is not covered by the initial fee and would become the responsibility of the patient to pay.

We will provide a detailed explanation and cost breakdown for the multi-day evaluation process. The evaluation fee is refundable if a cancellation is made at least 48 hours in advance of the appointment. A detailed explanation of our refund guarantee policy is included in a document you will receive once you decide to have the evaluation.

We can provide a map and lodging information prior to your visit if you desire.

You must contact us prior to any plans to have the evaluation procedure and must receive a formal confirmation either by phone, fax or letter. All additional required information will be provided at that time.
